This webform is for use by students registered with our Student Accessibility Services department (SAS) who have a current Accommodation Plan in place.

NOTE: This Test/Exam Accommodation Request needs to be submitted AT LEAST 6 days in advance of the scheduled date of the test or exam.  To book a test with less than 6 days notice, please visit or contact Student Accessibility Services (SAS).
