Booking a Test in the Testing Centre

You can book your Test Accommodations and Missed Tests/Rewrites through the Testing Centre Booking Form.

Testing Centre Booking Form

Please follow the instructions on the form to ensure you book the correct type of test.  If you do not have an active Accommodation Plan with Student Accessibility Services, please answer “no” to the first question.
If you have a testing request that falls outside of Test Accommodations or Missed/Rewrite tests, such as a Mature Student Assessment, please email the Testing Centre directly at [email protected]

Accommodated Testing

Students who face physical, medical, sensory, mental health or learning barriers to education may be eligible to receive accommodations for their tests. Accommodations may include, but are not limited to, additional time or use of adaptive technology. Students are encouraged to connect with an accessibility advisor to discuss their individual needs.

Student Accessibility Services (SAS) is the disability service office for Confederation College students. SAS provides academic accommodations for students with permanent and temporary disabilities/medical conditions in accordance with the Ontario Human Rights Commission (OHRC), the Accessibility for Ontarians with Disabilities Act (AODA) and Confederation College Policies.

To receive academic accommodations, students are required to self-identify their needs directly to SAS.   This involves providing SAS with medical documentation that outlines the functional limitations of a disability/medical condition.

The accommodations that a student receives (i.e. Accommodation Plan) will be based on the functional limitations identified in the medical documentation.

Scheduling an accommodated test (students with an Accommodation Plan)

As an accommodated student, you must book all your tests, quizzes and final exams via our Online Form. Accommodated students must write their test, quiz and final exam at the campus which their class is registered/attended. Your test must be booked for the same day and time as your class.

 Who books it? The student.

  1. Submit the Student Accommodation Test Request Form (minimum 5 business days before your test).
  2. Your instructor receives an automated prompt to add test details and instructions.
  3. The Testing Centre will ensure arrangements are made.
  4. On the Test date Students in Thunder Bay will check in at the Student Success Office (SHU 153).
If your test falls outside of regular office hours (8:30 am – 4:30 pm) please proceed directly to the Testing Area in the 1 floor library (SHU 146).

Missed an accommodated test? Get instructor approval, then submit the Student Accommodation Test Request Form again to rebook.

Missed or Re-write testing

A missed and/or make-up test occurs when a student is unable to write a test within the scheduled class time. Faculty may grant approval to students wishing to write a missed test based on the circumstances outlined in Confederation College’s Writing at Alternative Times policy.

Important notice:

When there are multiple students that miss the test/exam due to inclement weather or similar situations, the academic area is responsible for arranging the testing for students to make up the test/exam. Students are not to be directed to Testing Services. Students who do contact Testing Services will be advised to contact their academic area for information on the rescheduling of tests or exams.

If the academic area requires assistance booking a proctor for a classroom rewrite for a large group, staff or faculty in that area should contact the Testing Centre directly.

Booking a missed test or rewrite (non-accommodated students)

Schedule a missed or make-up test

Please ensure you have received confirmation and approval from faculty before scheduling a missed and/or re-write test. It is the student’s responsibility to connect with faculty and get approval for a such test before booking with testing services.

The process:

  1. Discuss your situation with your instructor; confirm if a rewrite is permitted.
  2. Submit the Testing Centre Booking Form, after confirming that a rewrite is possible with your instructor.  If you do not have an Accommodation Plan with Student Accessibility Services answer “No” to the first question on the form.
  3. When your instructor approves, they’ll provide instructions and set the last possible write date. When submitted by faculty, the student will receive confirmation that the test is approved.
  4. The student can contact the Testing Centre to schedule.
  5. If you have an accommodation plan, always use the Accommodated Test Request Form (even for rewrites).
