FIPPA has two main principles:

  1. Access to Information; and
  2. Privacy

FIPPA recognizes the College need to collect personal information to carry out its mandate. Confederation College will collect personal information that relates directly to, and is necessary for, operating the programs and administrative activities of the College.

Our Responsibilities

The College is required to collect personal information in accordance with the rules set out under FIPPA. We must ensure that privacy interests of applicants, students, and employees are not compromised by putting into place departmental procedures to ensure our obligations under FIPPA are met.

Use of Personal Information

The College must use personal information only for the stated purpose for which it was collected; or for a consistent purpose.

Disclosure of Collecting Personal Information

The College must inform an individual when collecting personal information, why the information is being collected; and if it will be shared and with whom.

Care and Control of Information – General Guideline

  1. Collect/record only information you need;
  2. Be objective and factual in what you record. Avoid recording unsubstantiated or subjective comments; and
  3. Mark records containing information requiring protection as CONFIDENTIAL and treat accordingly.

The privacy officer has primary responsibility for administering FIPPA. Formal requests under FIPPA must be directed to the Privacy Officer:

Collection Notice – Student

Student Collection Notice Disclaimer

If the information that you are collecting is going to be shared with other departments an additional statement must be included in the disclaimer disclosing the details of what will be shared and with whom.

Standard Collection Notice for Student Forms

Below is the standard Collection Notice that can be copied and pasted to forms that are used to collect personal information of STUDENTS, and originate from your department:

Personal information on this form is collected in accordance with sections 21, 39 and 43 of Freedom of Information and Protection of Privacy Act and under the legal authority of the Ministry of Training, Colleges and Universities Act, R.S.O. 1990, and the Ontario Colleges of Applied Arts and Technology Act, 2002, Regulations 34/03, and used for educational, administrative and statistical purposes of the College and/or Ministries and agencies of the Government of Ontario and the Government of Canada. Administrative purposes may include the disclosure to or on behalf of Student Union of Confederation College Inc (SUCCI) for the purposes of the activities of SUCCI, or to establish qualifications for benefits such as Drug Plan Card, U-Pass, and Foundation and Alumni activities, or for Key Performance Indicators (KPI) Survey. The College publishes the names of students who graduate and/or achieve academic excellence. Should you have any questions concerning your personal information, please contact Registrar at (807) 475-6612.

Instructions:

  • Place disclaimer at the bottom of all forms that contain personal information;
  • Fill in the section to specify the purpose for collecting the personal information; and
  • Send all forms to Human Resources for review prior to implementation.
