Note: All Affidavits must be sworn in front of a Commissioner of Taking Affidavits. Students will find a Commissioner at their local court house.

The following forms are available from the OSAP website. Please click on the form name to be brought to the OSAP website and then search for the form by the form name.
If you are returning to school full time, but will not be applying for new OSAP funding, you will need to advise the National Student Loans Service Centre that you are once again a full time student and want to return your loans to Interest Free Status. You can now submit an electronic "Continuation of Interest Free Status" form by logging into the OSAP web site and click on Continuation of Interest Free Status - start a new app. .
If you are having difficulty repaying your student loans, a number of options are available to you. Please refer to the Government of Canada - Student Financial Assistance website or call the National Student Loans Service Centre at 1-888-815-4514 for assistance.
